MP NEET PG Counselling 2026: Proformas, Bonds and NRI Affidavits Released — Rs 30 Lakh Seat-Leaving Bond

MP DME has released the document-scrutiny proforma, seat-leaving and rural service bond formats, NRI affidavits and undertaking for state NEET PG counselling 2026.

The Directorate of Medical Education, Madhya Pradesh, has published the set of proformas and affidavit formats candidates need for admission through the MP State Combined NEET PG Counselling 2026. The forms refer to the MP Medical Education Admission Rules 2018 as amended up to 30 September 2026.

Proforma 1 is the document-scrutiny form, with a checklist of originals to produce at admission. Proforma 2(a) is the MD/MS seat-leaving bond: leaving a Government autonomous college seat after the permitted time costs Rs 30 lakh, while in a private college the candidate must pay fees for the remaining course. Proforma 2(b) is the rural service bond — one year of service for non-in-service candidates (Rs 10 lakh if not served) and five years for in-service candidates (Rs 50 lakh). Proforma 2(c) covers in-service doctors in post-MBBS DNB and PG Diploma courses at district hospitals.

Proformas 3, 4 and 5 are notarised NRI/OCI affidavits and sponsorship certificates for NRI-quota seats in private colleges (Rs 1,000 stamp paper), and Proforma 6 is an undertaking on stipend, fees and discipline. Download the forms from dme.mponline.gov.in and prepare them before reporting.
